The molecules, the manufacturing, and how compounded GLP-1s differ from brand-name versions.

Semaglutide

The active pharmaceutical ingredient in Ozempic, Wegovy, and Rybelsus — a GLP-1 receptor agonist administered weekly by injection or daily as an oral tablet. It is the same molecule whether branded or compounded.

Why it matters: Understanding that semaglutide is the molecule — not the brand — helps clarify why compounded versions contain the same therapeutic agent as brand-name products.

Tirzepatide

The active pharmaceutical ingredient in Mounjaro and Zepbound — a dual GIP and GLP-1 receptor agonist administered weekly by injection. The dual mechanism produces greater average weight loss than semaglutide in clinical trials.

Why it matters: Tirzepatide is currently the most potent approved weight loss medication available.

Compounded Medication

A medication prepared by a licensed compounding pharmacy for a specific patient using pharmaceutical-grade ingredients. Compounded semaglutide contains the same active molecule as Ozempic or Wegovy but is not manufactured by Novo Nordisk and is not approved as a finished drug product.

Why it matters: Compounding allows access to the same therapeutic molecule at a fraction of brand-name prices when prepared by a licensed pharmacy.

Active Pharmaceutical Ingredient (API)

The actual drug molecule in a medication — the part that produces the therapeutic effect. In compounded semaglutide, the API is semaglutide itself, the same molecule found in Ozempic and Wegovy.

Why it matters: When evaluating compounded GLP-1 quality, the source and purity of the API is what counts — not the brand name on the label.

Peptide

A short chain of amino acids. Semaglutide and tirzepatide are both peptide-based medications, which is why they must be injected — peptides are broken down by stomach acid and cannot be absorbed effectively as oral medications at standard doses.

Why it matters: This explains why GLP-1 medications are weekly injections rather than daily pills, though oral semaglutide at higher doses is now available.

PCAB Accreditation

Pharmacy Compounding Accreditation Board certification — a voluntary quality credential demonstrating that a compounding pharmacy meets rigorous safety and quality standards verified by independent audit.

Why it matters: PCAB accreditation is the clearest independent signal of compounding pharmacy quality — always ask if your GLP-1 provider's pharmacy partner is PCAB-accredited.

Bacteriostatic Water

Sterile water containing a small amount of benzyl alcohol, used to reconstitute compounded GLP-1 medications in vial form. The benzyl alcohol prevents bacterial growth and extends shelf life.

Why it matters: Using the wrong diluent can degrade your medication — always use bacteriostatic water unless your provider specifies otherwise.

Off-Label Use

Prescribing a medication for a purpose, dose, or population not listed in the approved label. Ozempic is frequently prescribed off-label for weight loss even though Wegovy is the approved weight loss version of the same molecule.

Why it matters: Off-label prescribing is legal and common — it does not mean the use is unsafe or unsupported by clinical evidence.

How GLP-1 dosing works — from your first injection to long-term maintenance.

Dose Escalation

The structured process of gradually increasing GLP-1 medication from a low starting dose to a therapeutic maintenance dose, typically over 12–20 weeks. This allows the GI tract to adapt and minimizes nausea and vomiting.

Why it matters: Skipping escalation steps dramatically increases side effect severity — the schedule exists for good reason.

Maintenance Dose

The final therapeutic dose of GLP-1 medication reached after completing the escalation schedule. For semaglutide this is typically 2.4mg weekly. For tirzepatide it ranges from 10–15mg weekly depending on response and tolerance.

Why it matters: Clinical trial weight loss results are measured at maintenance dose — results during escalation are typically lower.

Non-Responder

A patient who loses less than 5% of body weight after 12–16 weeks at maintenance dose. Approximately 10–15% of GLP-1 patients are non-responders. Non-response is usually addressed by switching medications.

Why it matters: If you are not losing weight after 3–4 months at full dose, your provider should evaluate whether a medication switch is appropriate rather than continuing an ineffective treatment.

How GLP-1 medications actually work inside your body.

GLP-1 (Glucagon-Like Peptide-1)

A hormone naturally produced in your gut after eating that signals fullness, stimulates insulin release, and slows digestion. GLP-1 medications mimic and amplify this natural hormone.

Why it matters: This is the foundational mechanism behind all semaglutide and tirzepatide medications.

GIP (Glucose-dependent Insulinotropic Polypeptide)

A gut hormone that works alongside GLP-1 to regulate insulin secretion and fat metabolism. Tirzepatide activates both GIP and GLP-1 receptors simultaneously.

Why it matters: The addition of GIP agonism is the key pharmacological reason tirzepatide outperforms semaglutide in weight loss comparisons.

Dual Agonist

A medication that activates two hormone receptors simultaneously. Tirzepatide is a dual GIP and GLP-1 receptor agonist, which is why it produces greater average weight loss than semaglutide in head-to-head comparisons.

Why it matters: The dual mechanism is the pharmacological reason SURMOUNT-1 trial results exceeded what was seen in semaglutide trials.

Incretin

A class of gut hormones including GLP-1 and GIP that stimulate insulin secretion in response to food intake. GLP-1 medications are sometimes called incretin mimetics.

Why it matters: The incretin system is why GLP-1 medications lower blood sugar without causing dangerous hypoglycemia in non-diabetic patients.

Gastric Emptying

The rate at which your stomach moves food into the small intestine. GLP-1 medications slow this process, which extends fullness signals and reduces post-meal blood sugar spikes.

Why it matters: Slowed gastric emptying is both why GLP-1 medications are so effective and why high-fat meals cause discomfort on medication.

Insulin Resistance

A condition where cells don't respond normally to insulin, requiring the pancreas to produce more to maintain normal blood sugar. It is the underlying mechanism of prediabetes and type 2 diabetes.

Why it matters: GLP-1 medications directly improve insulin sensitivity — one reason they are effective for both weight loss and diabetes management.

Food Noise

The weight range your body actively defends through hormonal and metabolic mechanisms. GLP-1 medications are thought to lower the defended set point by reducing the hormonal drive to regain weight.

Why it matters: Set point theory explains why diet and exercise alone often fail long-term — and why pharmacological intervention may be necessary for sustained results.

Adaptive Thermogenesis

BMI (Body Mass Index)

A number calculated from height and weight used as a screening tool for weight classification. BMI 18.5–24.9 is normal, 25–29.9 is overweight, 30 or above is obese. GLP-1 medications are typically prescribed for BMI 30+ or BMI 27+ with a weight-related condition.

Why it matters: BMI is the primary eligibility criterion for GLP-1 prescription — though it has known limitations as a measure of metabolic health.

A1C (HbA1c)

A blood test measuring average blood sugar over the past 2–3 months, expressed as a percentage. Normal is below 5.7%. Pre-diabetes is 5.7–6.4%. Type 2 diabetes is 6.5% and above. GLP-1 medications reduce A1C by 1.5–2.3%.

Why it matters: A1C reduction is one reason GLP-1 medications are prescribed for both diabetes management and weight loss.

Lean Mass

The weight of everything in your body that isn't fat — muscle, bone, organs, and water. Preserving lean mass during weight loss is critical for long-term metabolic health and physical function.

Why it matters: Approximately 25% of weight lost on GLP-1 medications is lean mass — protein intake and resistance training are essential to minimize this.

Visceral Fat

Fat stored around internal organs in the abdominal cavity. More metabolically dangerous than subcutaneous fat, visceral fat is strongly associated with insulin resistance, cardiovascular disease, and systemic inflammation.

Why it matters: GLP-1 medications preferentially reduce visceral fat — patients often notice waist circumference changes before the scale reflects significant total weight loss.

Sarcopenia

Age-related muscle loss that accelerates after age 50. GLP-1 medications can compound natural muscle loss if protein intake and resistance training are not prioritized during treatment.

Why it matters: Women over 50 on GLP-1 therapy face the highest sarcopenia risk and should prioritize 1.2–1.6g of protein per kilogram of ideal body weight daily from day one.

Telogen Effluvium

Temporary hair shedding triggered by physiological stress including rapid weight loss. It typically begins 2–4 months after significant weight loss starts and resolves within 6–12 months. It is not a direct drug effect.

Why it matters: Hair thinning on GLP-1 medications is almost always temporary and related to weight loss rate — not a permanent side effect of the medication itself.

Comorbidity

A condition that exists simultaneously with another primary condition. For GLP-1 eligibility, relevant comorbidities include type 2 diabetes, hypertension, dyslipidemia, obstructive sleep apnea, and cardiovascular disease. Having one or more of these with a BMI of 27 or above qualifies a patient for GLP-1 prescriptions.

Why it matters: Patients with a BMI below 30 who have weight-related health conditions are eligible for GLP-1 medications — many patients are unaware that the 27+ BMI threshold applies.
