2026 Compounded Tirzepatide Price Report: What Seven Online Providers Charge

Published October 27, 2026

Tirzepatide pricing pages rarely show the same thing twice. One lists a monthly figure that only applies if you prepay a year. Another bills every four weeks, which adds a thirteenth charge to the calendar. A third sells the medication by the dose and the clinician visit as a separate membership. This report puts seven online providers of compounded tirzepatide on one footing: the regular price each one published on October 19, 2026, with no promo code, on a plan that commits you to three months or less, and the amount that would leave your card.

Ozari Health's own plans appear below on the same terms as everyone else's.

Twelve-month totals assume the plan's charge holds for a year and use the starting-dose price where a provider prices by dose. Ozari's pricing was last verified on its site on September 30, 2026; every other figure comes from the provider's own pages as they read on October 19, 2026.

What the numbers say

The lowest regular monthly price on a plan of three months or less runs from $125 to $199. The spread looks modest until you multiply it out: over a year, the same seven plans run from $1,500 to $2,587, a gap of more than $1,000 for the same medication category.

Month-to-month billing costs more almost everywhere. Among the providers that sell a monthly plan without a membership, the rate runs from $129 to $219. Ozari Select, Ozari's monthly plan, is $235 per month, and its quarterly rate is $199. The quarterly or three-month plan beat the monthly price at every provider here that offers both.

Prepaying a year lowers the monthly figure further at several providers, to between $139 and $149 a month, but it means $1,668 to $1,788 up front and refund terms that differ from one company to the next. Ozari Select's annual rate is $140 a month, with early cancellation refunded less the months used at the standard monthly rate.

Three things that move the tirzepatide price

The billing cycle. A plan billed every four weeks charges 13 times a year. Sprout's $199 charge comes to $2,587 over 12 months, where a true monthly plan at the same figure would come to $2,388.

A membership fee. Mochi Health splits the bill into a membership ($79 a month, or $207 per three months) and medication priced by the dose, from $90 a month for tirzepatide. The $159 starting total is a floor. Every other provider in this report folds the clinician visit into one price.

Dose pricing. Five of the seven providers publish one price that holds from the starting dose to the highest. Mochi prices each dose step, and Kongo's pages show one price per plan without saying what happens at higher doses. If a provider shows only a starting price, ask what your target dose costs before you pay.

How Ozari's plans fit

Ozari One prices compounded tirzepatide with B12 at $125 a month, billed $375 every three months, the lowest regular monthly figure in this report, and the price does not change as the dose increases. It bills quarterly only, comes in one formulation, and is available in every state except California. Ozari Select, available in all 50 states, offers tirzepatide with B12, with glycine or with no additives at $235 monthly, $199 quarterly, $175 semi-annually and $140 annually. Both plans include the clinician visit, the medication and free expedited shipping, charge no membership fee, and refund the full payment if a clinician does not prescribe. Prescriptions are filled by US-licensed 503A compounding pharmacies named on the pharmacy standards page, and the label on every vial identifies the pharmacy that prepared it.

About this report

Prices are each provider's published cash-pay rates for compounded tirzepatide, collected from its own website on October 19, 2026, excluding promo codes and limited-time offers. FDA-approved GLP-1 options are outside the report's scope. Providers change prices often, so confirm the current total at checkout. This report will be updated as pricing changes.

Compounded medications are not FDA-approved and have not been evaluated by the FDA for safety, effectiveness, or quality. A licensed clinician decides whether compounded tirzepatide is appropriate for an individual patient.

Frequently Asked Questions

What is the cheapest compounded tirzepatide online in October 2026?

On a plan of three months or less and no promo code, the lowest regular monthly rate across the seven online providers we checked on October 19, 2026 was $125 per month at Ozari One. The seven published figures ran from $125 to $199 per month, with 12-month totals from $1,500 to $2,587.

Why does the same medication cost so much more at one provider than another?

Three structures move the number. A plan billed every four weeks adds a thirteenth charge each year. A separate membership fee splits the bill into two line items. And dose pricing raises the price as your dose rises. Flat quarterly plans without a membership were consistently the lowest among the seven.

Does the advertised monthly figure include everything?

Not always. Five of the seven providers quote one price that covers the clinician visit, the medication and shipping. One advertises a "from" figure that is a membership plus a dose-tiered medication price, and one bills every four weeks rather than monthly. Check the billing cycle, the membership and the dose policy before you compare two headline numbers.

Is compounded tirzepatide the same as Mounjaro or Zepbound?

No. Mounjaro and Zepbound are FDA-approved branded tirzepatide products from Eli Lilly. Compounded tirzepatide is prepared by a licensed US pharmacy under an individual prescription and is not FDA-approved; the FDA has not evaluated it for safety, effectiveness, or quality.

When were these prices checked?

Every price in this report was collected from the provider's own pricing page on October 19, 2026, with no promo codes applied. Ozari Health's rates were last verified on its own site on September 30, 2026. Pricing changes often, so confirm the current total at checkout.
